As the Field Home Care Lead Home Health Aid Manager, you will play a key role in managing and supervising home health aides and personal care assistants who provide essential support to individuals in their homes. Your leadership, organization, and compassion will contribute to the delivery of high-quality, personalized care that enhances the well-being and independence of clients within the home care setting.

Responsibilities:

  • Supervise and lead a team of home health aides and personal care assistants, providing guidance, support, and ongoing training to ensure compassionate and competent care delivery.
  • Conduct client assessments to determine care needs, preferences, and individualized care plans that promote the safety, comfort, and dignity of the clients.
  • Coordinate and assign client care schedules to home health aides, ensuring continuity of care, appropriate staffing levels, and consideration of client and staff preferences.
  • Monitor and evaluate the performance of home health aides, providing constructive feedback, coaching, and recognition for exceptional care provision.
  • Collaborate with healthcare professionals, including nurses and therapists, to support the implementation of care plans and address clients' changing needs and conditions.
  • Ensure compliance with care protocols, safety standards, and regulatory requirements governing home health aide services.
  • Conduct home visits to observe the quality of care, assess client satisfaction, and address any concerns or changes in care needs.
  • Serve as an advocate for clients and their families, providing guidance, resources, and support to address their home care needs and concerns.

Qualifications:

  • High school diploma or equivalent; additional certifications, training, or coursework in home health aide services or healthcare management are preferred.
  • Proven experience as a home health aide or personal care assistant, with a deep understanding of the needs and challenges associated with providing care in the home setting.
  • Leadership or supervisory experience within a home care agency or similar environment, demonstrating the ability to mentor, motivate, and lead a dedicated caregiving team.
  • Excellent interpersonal and communication skills, with the ability to interact with clients, families, and healthcare providers in a compassionate and professional manner.
  • Knowledge of safety and infection control practices, as well as the ability to provide guidance and training on client care techniques and procedures.
  • Compassion, empathy, and a genuine commitment to enhancing the well-being and quality of life for clients in need of home health aide services.
  • Familiarity with electronic health and scheduling systems, as well as proficiency in using technology to facilitate care coordination and documentation.
  • Flexibility and adaptability to address changing client care needs, schedules, and the dynamic nature of home care services.
