As the Field Assessments Manager for Home Health Care, you will be responsible for overseeing and managing the assessment process for home health care patients to ensure the delivery of personalized, high-quality care. Your leadership and expertise in patient evaluation and care planning will contribute to the effective coordination of services and the optimal well-being of patients receiving home health care.

Responsibilities:

  • Lead and supervise a team of assessment coordinators, guiding their efforts to conduct comprehensive evaluations of patients' health, functional status, and care needs within the home setting.
  • Implement standardized assessment tools and protocols to gather essential information about patients, including medical history, current health status, medication use, and social determinants of health.
  • Collaborate with healthcare providers, including physicians, nurses, therapists, and other care team members, to ensure accurate and holistic assessments that inform the development of individualized care plans.
  • Monitor the timeliness and quality of assessments, ensuring that all patients receiving home health care services undergo thorough evaluations to support optimal care delivery.
  • Analyze assessment data and findings to identify trends, clinical concerns, and opportunities for improvement in care planning, interventions, and patient outcomes.
  • Oversee the coordination of assessments with scheduled care visits, facilitating seamless transitions between assessment, care planning, and the implementation of care services.
  • Ensure compliance with regulatory standards, documentation requirements, and privacy regulations governing patient assessments and care planning in the home health care setting.
  • Serve as a resource and consultant to assessment coordinators, care team members, and leadership, providing expertise on assessment best practices, care coordination strategies, and patient-centered care delivery.

Qualifications:

  • Current licensure as a Registered Nurse (RN) or other clinical healthcare professional with experience in patient assessments, care coordination, or case management.
  • Proven leadership and managerial experience in a healthcare setting, with a focus on patient assessments, care planning, or care coordination, preferably within home health care or a community-based care environment.
  • Comprehensive knowledge of assessment tools, clinical documentation standards, and care planning processes relevant to home health care and community-based care delivery.
  • Strong communication and interpersonal skills, with the ability to collaborate with interdisciplinary care team members, patients, and their families to support comprehensive and person-centered care.
  • Experience with electronic health record systems, data analysis, and performance improvement methodologies to enhance the quality and effectiveness of patient assessment and care coordination.
  • Commitment to upholding ethical standards, privacy regulations, and confidentiality in handling patient information and assessment data.
  • Flexibility and adaptability to address the unique needs and challenges associated with assessing and coordinating care for patients receiving home health care services.
